Retention Contract Form

In a concerted effort to assist students in achieving “satisfactory” academic standing, an academic contract is created between the student and the University. Contract students’ academic progress is reevaluated at the end of each semester. Students must attain satisfactory academic progress to be reviewed for removal from contract.

Upon submission of the contract, additional information relative to weekly activities will be e-mailed to your Hornets e-mail account. You must utilize your Hornets e-mail account to receive your weekly objectives. Objectives vary by week and may include but are not limited to the following: self-report course progress updates, instructor progress reports, individual meetings and referrals for utilization of campus resources.
